What is thyroid?

Thyroid gland is a gland which is present at the front side of the neck. Adam’s apple is present just above the thyroid gland. Thyroid gland secretes hormones called T3 and T4 hormones. These are responsible for regulating the metabolic rate of the body, controlling digestion, heart and muscle function, brain development and also maintaining bone health. So, any irregularities in this gland may cause imbalance in the whole body function.

Thyroid gland disorder in children:

It’s all in the right amount of hormone secretion by the thyroid gland. If it secretes too much thyroid hormones, the condition is called hyperthyroidism and when it secretes too little hormones, it will result in a condition called hypothyroidism in kids. Both of these disorders can affect the child’s growth, mood, health, learning ability and concentration. Hashimoto’s thyroiditis (HT), is a disease which typically causes hypothyroidism, is the most occurred thyroid disease in children.

Symptoms of thyroid in kids: 

These most common symptoms listed here can help to relate if your child is having thyroid disorder.

  1. Hypothyroidism: 

In the case of kids with hypothyroidism, their thyroid gland is producing either too little hormones, a condition known as primary hypothyroidism, or the thyroid gland is not stimulated properly, a condition known as secondary hypothyroidism. Symptoms include constipation, rough dry skin, slow growth, fatigue, cold intolerance, bruising, too much sleeping, bone fracture, delayed puberty etc. Sometimes, the child with thyroid issue is overweight or obese and shorter in height.

  1. Hyperthyroidism: 

In case of hyperthyroidism, the thyroid gland produces high amounts of hormones which may be because of overactivity or overstimulation of gland. The symptoms include, shorter attention spans, tremors, weight loss, emotional breakdowns, increased appetite, goitre(enlarged thyroid), less blinking, upper eyelid lag, excessive sweating, muscle weakness,high blood pressure etc.

DISEASES ASSOCIATED WITH THYROID IN KIDS:

Thyroid gland disorder can also cause other endocrine disorders such as tumours in pituitary gland, diabetes mellitus, and tumours in adrenal gland.   1. Soybean and soybean products: 

Soybean contains isoflavones, compounds that negatively affect the thyroid affected child. It may increase the risk of hypothyroidism. It interferes with the absorption of thyroid medication.

  1. Cruciferous vegetables:

Cruciferous vegetables include brussel sprouts, cabbage, cauliflower, broccoli, kale, turnip etc. These are definitely full of nutrients and fibre rich but they may affect the thyroid’s ability to produce the hormones, especially if your child is iodine deficient.

  1. Gluten:

Gluten found in some of the cereals such as wheat and rye, and their products may hamper the absorption of medication given for thyroid in the small intestine. Consuming fibre rich foods is good. However, too much fibre consumption in terms of whole grains, vegetables, fruits, beans and legumes may affect the absorption of medication in the intestine.

Foods to avoid with hyperthyroidism

Too much iodine consumption can worsen hyperthyroidism. Hence, children with hyperthyroidism should avoid or limit eating iodized salt, fish and shellfish, dairy products, egg yolk, seaweed, baked goods and products with soy, gluten and caffeine.

Ashwagandha:

What are motor tics or muscle spasms?

Motor tics are unintentional muscle movements caused by spasm like contraction which usually involves muscles of the face, eyes, mouth, neck, head and even shoulders.

Tics may look like intentionally done, but they are not. Although a person or a child suppresses the tics for a short time, tics and sound repeat and the urge will become stronger.

How to know whether your child has muscle spasm?

Motor tics may be a normal or complex. If it’s normal, only one muscle or body part is involved which causes no harm. But, it may be painful or embarrassing at that moment.

The common symptoms of normal or simple motor tics include:

  • Eye blinking or eye jerking quickly
  • Tongue sticking out
  • Head jerk or twitches
  • Muscle spasm
  • Hopping and squatting
  • Shoulder shrugs

Whereas, a combination of different types of simple motor tics affects different muscle groups at a time, and is called complex motor tics. These are usually slower and it seems the person is doing movements intentionally.

The common symptoms of complex motor tics or muscle spasms include:

  • Pouting of face
  • Repeatedly touching people or things
  • Revolving movement of hands

Similar to motor tics, vocal tics also may occur either simple or complex. 

Simple vocal tics include,

  • Noisy breathing
  • Hissing and barking
  • Snorting
  • Smelling
  • Clearing the throat.

Complex vocal tics include,

  • Words
  • Phrases
  • Sentences
ADHD and muscle spasms:

ADHD is a disorder which is very complex and it involves different effects and symptoms. It does not directly cause the motor tics but children with ADHD can sometimes get co-occurrence of motor tics.

ADHD: Multiple disorders

Children who have ADHD can have multiple disorders at the same time. Researchers found that  about 50-70% of the children with ADHD have motor tics.

There are some disorders involving muscle spasm or muscle cramps which occur in association with ADHD:

  1. Tourette syndrome (TS): People with TS sometimes experience ADHD symptoms. This could be near 90%. Some of the studies also show that, for children with TS, the tics tend to reduce in their teenage years.
  2. Obsessive-compulsive disorder (OCD):  Researchers have found that people whose OCD symptoms include aggressive compulsions and symmetry behaviours most commonly have tics.
  3. Autism spectrum conditions. Some of the symptoms associated with autism are body, face, and voice tics. Most of the children with ADHD have autism.

What can you do about motor tics (also with ADHD)?

  1. Improving sleep life: When in fatigue, tics may get worse. Getting proper sleep may improve the condition. This may be difficult for ADHD kids as sleep issues are very common with them.
  2. Reduce stress and anxiety: Anxiety and stress may worsen the condition of tics. In this case, tics may act as a result of stress and anxiety. Regularly doing meditation, yoga and physical activity may improve the situation.
  3. Educate the circle: It’s always good to inform the teachers, caretakers, peers and coaches about the condition your kid is suffering through. This will create a supportive environment for the kid.
Foods to avoid: 
Refined sugar:

Excessive sugar impairs cognitive skills as well as self control. Moreover, sugar damages the protein in our body, resulting in ageing of organs, which further compromises immunity, growth and functioning. Greatest source of sugar in the average kid’s diet are sugary snacks and beverages like cookies, cakes, candies, fruit juice, soda, and sports drinks. Everything that the child picks up to eat today has strains of added sugar in some form or the other.

Preservatives:

Most of packaged food companies are adding artificial preservatives to delay spoilage and contamination in foods.  Artificial preservatives such as nitrates, benzoates, sulfites, sorbates, parabens, formaldehyde, BHT, BHA and several others can cause serious health hazards such as hypersensitivity, allergy, asthma, hyperactivity, neurological damage and muscle spasms.

Refined oil:

The most evident reason for bloating is constipation. Apart from that, indigestion and hormonal imbalance may also result in bloating of stomach.

Causes of bloating of stomach:

1. Eating gassy foods:

Some of the legumes, millets and cereals are rich in oligosaccharides such as raffinose and stachyose. These oligosaccharides are sometimes not properly digested in stomach and small intestine due to high levels of insoluble fibre and they pass to the large intestine where they are fermented by gut bacteria to produce gas. Because of this, the person feels bloated after eating such foods.

2. FODMAP:

FODMAPs refer to a group of carbohydrates that are not digested easily in some people. They may result in fluid build-up and gas trouble. FODMAPs include fructose present in honey, lactose in dairy products, beans and lentils, gluten-containing foods and also some foods such as apples, pears and cherries.

3. Eating too fast:

If you are a person with the habit of gulping down food too fast, you are more likely to have bloating frequently. When you engulf the food quickly, you swallow more air which may pass to your intestines and cause bloating.

4. Consuming fizzy drinks:

When you consume more fizzy drinks loaded with carbon dioxide gas, it may fill in your belly. Some of the gas is released through burping. The remaining gas may fill in tummy and pass to the intestines and even result in anal gas.

5. Drinking too little water:

When you consume fibre-rich foods and don’t drink enough water, it may tighten the stools and result in constipation. Due to constipation, you feel your belly is bloated and often feel pain in your stomach.

6. Eating junk foods:

7. Not moving enough:

Having a big and hearty meal is so satisfying right!! Yes. There’s no harm in having a big meal often. But if you are a person who goes to bed right after your meal, then you may suffer from indigestion and gastric problems ultimately leading to stomach bloating. How long does it take bloating to leave?

What are hind milk and fore milk?

Hindmilk and foremilk are not at all separate milk. These terms indicate the time of feeding whether in the beginning or end of feeding the baby. The breast milk that the infant receives at the beginning of a nursing session is referred to as foremilk; it contains less fat and more water. As breastfeeding continues, the amount of breast milk decreases and its fatty content steadily rises. The hindmilk is the breast milk that is produced at the end of the nursing session and has a lower volume but a much higher fat content.

What is hind milk and foremilk imbalance?

Hindmilk and foremilk imbalance is a condition where breast milk does not have a good mixture of hindmilk and foremilk for feeding the baby.

Usually, the mother’s breast produces only one type of milk which is naturally rich in fat. But, as the feeding is continued, the amount of fat in the breast milk varies. When the milk is produced in the breast, the globules of the fat in the milk stick to each other and move towards the walls of the alveoli. When there is a gap between the feedings, the milk gets collected inside the breast and then slowly moves towards the nipples leaving behind the fat in the milk ducts. When there is more gap between the two feedings, the fat content in the milk reduces in comparison with the fat content at the beginning of the feeding.

Causes of hind milk and foremilk imbalance:

When the baby is breastfed in the proper manner, it receives a good mixture of hindmilk and foremilk. However, the baby when too hungry and quickly drinks the milk, or when the baby switches between breasts, there will not be proper mixing of foremilk and hindmilk and as a result, the baby consumes more foremilk than the hindmilk.

Symptoms of hindmilk and foremilk imbalance:

1. Baby when passes blood in poop:

When there is foremilk and hindmilk imbalance while baby feeds, there is more pressure on the digestive system. They may pass gas which causes their sensitive anal area and intestine to be pressurized frequently. This might result in the blood spots occuring in the poop of the baby.

2. Regular and Repetitive Problems of digestion:

Milk imbalance usually causes digestion problems regularly. The major evidence for that is gastric problem. Babies may fart more often and burp many times in this case.

3. Reduced weight:

Breast milk forms a strong foundation for a baby’s nutrition as it facilitates weight gain and builds the fat necessary to keep the body warm and immune. But, as the hindmilk contains more of these nutrients than the foremilk, any imbalance in their composition may result in reduced weight of the baby.

4. Required to feed more frequently:

As the hindmilk has more fat, it fills up the stomach of the baby and makes him feel satisfied. If there is no hindmilk, the foremilk which is thin and light cannot satisfy the baby and the baby demands to be fed more and more.

5. Occurrence of diaper rashes:

The hindmilk and foremilk imbalance changes the whole milk’s nature. It may result in slightly acidic milk, resulting in difficulties in pooping, leading to diaper rashes.

6. Gastric Problems:

The baby may suffer from stomach pain due to gas, and sometimes colic attacks. Some babies release gas by farting and burping but others may not do that. As a result, the stomach muscles may tighten and spasm, and your child may tend to cry loudly, and may sleep in a foetal position to relieve the pain. It may also result in a swollen belly.

7. Babies may poop immediately after being breastfed:

In case of an imbalance of hindmilk and foremilk, there will be a larger amount of foremilk which hardly gives the stomach and intestines any time to do their work. Milk now passes directly forward, resulting in immediate bowel movement and poop.

8. Unnatural texture and nature of poop:

A Baby’s poop indicates how healthy the child is and whether or not the baby is receiving the right nutrition. Breast milk, if imbalanced, will contain more lactose than usual. The baby’s poop may get a green tint and liquid nature rather than solid.

Give Breastfeeding the Time and Patience it Deserves:

For most of the mothers and every baby, breastfeeding is a new experience. After many trials and errors, both will adjust to each other’s preferences, positions, and time schedules and finally end up on the one that works for both. So, it’s important for both baby and mother to be patient so that the breastfeeding experience is comfortable.

Pump Out a Little Milk Before Breastfeeding Your Baby:

Foremilk comes first before the fatty hindmilk. Hence, it’s important to pump out some of it before feeding the baby if it is getting full by the foremilk itself. This would frame a good balance of both foremilk and hindmilk, enabling the baby the right kind of nutrition.

Having a break in between breastfeeding cycles:

It’s common for many moms to finish one breast’s milk before switching to the other. This gap is much more before continuing the feeding by the other breast. It’s good to make the child burp and then giving him some rest ensures that he can feed for a longer time and mom can get a good proportion of hindmilk, too.

Trying different breastfeeding positions:

In cases of milk imbalance, lying on your side helps to provide a good flow of milk, also helps the baby to take a small nap in between feeding, and makes him drink more. Otherwise, you can lie down and place the baby on top of you or adopt an upright position of sitting, which helps to give the baby more control over feeding.
